Community Living

Community Living

Spring Dell provides a secure place for your loved one to feel safe and loved and become a part of our Spring Dell ‘family’. Our caregivers provide supports in small community living settings with three to four people in the home, helping each person grow, develop, and live life to their fullest. Our Community Living Program is committed to respecting people’s choices regarding housemates, staff, neighborhood, daily routines, and activities. Supports and services are created to be seamless for individuals from early adulthood through retirement and beyond. Everyone served here are the center of our planning process and services which emphasize maximum growth and independence for each person and the use of unlimited community resources. Our Community Living Supports includes transportation, personal care, assistance with medical appointments, shopping, meal planning and preparation, banking and budget assistance, nursing and psychological supports, daily skill development, home maintenance, and facilitation of community connections and other supports as determined in the Person-Centered Plan (PCP).

Shared Living

Spring Dell’s Shared Living services provides supports for persons with intellectual and developmental disabilities to build and keep independent living skills needed for everyday life, including money management, maintaining finances, assistance with transportation, medical appointments and community inclusion. The service supports individuals to share a home and life experiences with another person, or a family in the community that they choose. The shared living service is based on trust and commitment the individual and the family have with each other.

Supported Living

Spring Dell’s Supported Living services provide individualized support to help people with intellectual and developmental disabilities or other needs live independently in their own residence. Services are tailored to the individual’s specific needs and goals, with support plans developed to help them achieve their objectives. This can include assistance with daily living tasks, skill-building and/or accessing community resources. Spring Dell also promotes participation in community life helping individuals build relationships and access opportunities.

Respite Care

Respite Care provides planned or emergency relief for primary caregivers. This is a short-term break for caregivers who look after their loved one with intellectual or developmental disabilities. Respite care can provide much-needed support, whether you need services during an emergency, want a chance to recharge privately, or simply want to spend quality time with other family members. Taking care of your loved one is a very rewarding and enriching experience, but even the most loving caregivers sometimes need help. Spring Dell’s ‘Respite Care’ assists primary caregivers by providing loving staff to care for your loved one while you focus on other matters.

Transportation:

Spring Dell promotes independence by encouraging individuals to arrange their own daily transportation through community resources. All other options should be explored before requesting transportation services from Spring Dell, especially for those in the Employment/Community program. When space allows, round-trip transportation between home and community jobs may be provided.
